Application Form for Reception

Please complete this form if you wish to apply for a place at Morasha. The closing date for applications for the Reception Class in September 2012 is 17 December 2010.

In addition to this form, we will need:

1. a copy of your child's full birth certificate
2. proof of residence (council tax demand or utilities bill no more than 6 months old showing your current address)
3. a cheque for £25 to cover the costs of administration made payable to Finchley Jewish Primary School Trust

Please send these to Morasha Admissions, 37 Moss Hall Grove, London, N12 8PE as soon as possible after you have submitted your application form.
